But even more, as you described how you came to the list (or lists), I had a strong connection to the work I’ve done with “process improvement” under the Theory of Constraints banner. One of the things I look for when going into businesses is something called the “Undesirable Effects” that people are seeing in their businesses. Depending on what general business area (projects, manufacturing, supply chain, finance), there are a set of these Undesirable Effects that show up nearly everywhere. They are articulated many ways, but they can be narrowed down into a smaller set.

I suspect you find the same with the list you are creating, if I understand it correctly.

If this is correct, the next thing to do is to draw connections between the effects to demonstrate how the originate from a very small set of root causes – as you suggest above. Then the organization can focus on the root cause(s) instead of all the effects individually.
